This is a chart I use every year. It is perfect for ESL observations, guided reading notes, anecdotal notes when monitoring independent centers, tracking behavior and pretty much anything else you can think of!
This is a Word doc so it is totally editable. All colors, fonts, etc. can be changed. The design is all set so it will be easy to envision the final product. I create one set per class, make several copies and use them tons of ways.

Classroom Expectations Doodle Note Page
This doodle notes page is a great way to start the school year! Set your exptations from day one. There is a spot for the students to fill in the rules & also a side for them to think about why each expectation is there. Go Math Lesson Plan Template
This is a lesson plan template I created as an overview of the day's Go Math lesson. I thought it would also be helpful to leave for a sub. It includes the routines of the program as well as places to add notes for modifications or tips. This was designed based on fourth grade, but I believe the routines are similar for other grades, and it is in Word format so you can edit it as you see fit.

Math Assessment for Learning Template
The Assessment for Learning Template is originally an Ontario Ministry of Education document. I recreated it for a math team several years ago and have since added columns for Knowledge/Understanding, Thinking, Communication, and Application. This is useful for taking notes during three-part lessons and other small-group math work.

This Student Data Binder is a great way to have students track and own their own progress. Students can write their own mission and vision statements, rate their attitudes, chart math progress, log test scores and note strategies for continued success or improvement.
