I created this slip for students who have problems arriving on time, staying on-task and completing in class work. A staff member will fill out this form (This is a .doc. You can add/delete items in the document) with the student's name, parent/guardian's name, and fill out the student's classes, and teacher name and contact number. Students will be aware that the parent can call the teacher at any time, and the teacher can call the parent at any time because the numbers are clearly listed on
