The ABC Observation Form is a tool to document student behavior in real time. When observing, record the Setting/Context (where and with whom the behavior occurs), the Antecedent (what happened immediately before), the Behavior (the specific, observable action), and the Consequence (what happened right after). At the end, identify the Perceived Function (why the student may be engaging in the behavior, such as attention, escape, or access). This structured approach helps staff recognize pat
