This is a chart I use every year. It is perfect for ESL observations, guided reading notes, anecdotal notes when monitoring independent centers, tracking behavior and pretty much anything else you can think of! This is a Word doc so it is totally editable. All colors, fonts, etc. can be changed. The design is all set so it will be easy to envision the final product. I create one set per class, make several copies and use them tons of ways. Enjoy!
