This free DSA Student Profile template is an Excel spreadsheet for easy use. It's set up to automatically calculate your stage scores as you enter them for your kiddos.
If you want to have separate charts for each student, make copies of the template before you begin entering your scores. This is great for retaining the information and updating throughout the year.
