Chart to use based on Intervention Plans (weekly behaviour chart). Students, parents and teachers agree on what needs to improve (p.2) and then the teachers fill up the student's behaviour chart for everyday of the week. The parents sign the report and send it back to school. Booklet is made for 1 week, but p.3 & 4 can be copied as many times as needed based on the length of the IP.
