This is a form I have my band & choir students complete when they need a copy of something. It has worked well for me this year. It eliminates students coming up & telling me they need a copy. It puts the responsibility on the student to write it down. It also helps me keep track if a particular student continually loses music.

If you are like me, you get overwhelmed with students lined up at your desk needing help on an assignment. A waiting room eliminates that stress and allows for a student to know that their turn is coming without interruptions. You will need to purchase clothes pins.
