Retaking tests in math is an important option. This form has made a big difference for my students because it puts the responsibility back on them to get extra help prior to the retake. I also have connected it to homework completion and reflection. Students are asked to use resources like their text, the teacher, or the internet. It can be easily modified for whatever you feel is helpful for your students.
