First Quarter Self-Reflection Form
I created this form to do mid-term grade checks with students. The form includes areas of positivity for the student, growth they have witnessed within themselves, and a spot for them to talk about what is not working. I really like helping students set goals, so there are lines on the document to include goals, steps, and interventions to improve student success.
