Fast and easy 2 page form with two versions. I print them in bulk front and back so I just have one page case history forms I can give to parents or send home. Lots of check boxes because I've found that people who would otherwise entirely skip a question will often at least check a box. It's also easier for you if you're doing a phone intake.
