This data collection sheet provides a quick and easy way to take data during your therapy session. There is room for up to 5 goals per student, or 1 goal for 5 students in a group (such as an artic group). Each goal has up to 10 dates of entry, so you can record data for the full grading period, for 10 consecutive sessions, or for 10 random dates. If your patient has poor safety awareness but can be redirected, it may be beneficial to have a sign posted within sight to act as a visual cue. I made this sign so that I could quickly print, and provide a resource for patients who have a high risk for falling. This can also be utilized during therapy to increase safety awareness with spaced time retrieval and errorless learning.
