I differentiate by readiness using some sort of pre-assessment then I use this form to figure out how I will group the students the next day. Usually, the lowest group is with me doing some reteaching and the high group is doing more challenging versions of the assignment. The middle group is working together on the assignment and I check in with them as needed.

I created this tool to help my high school students who did not participate much in the classroom setting. Students set a goal and keep track of their participation. It really helped increase participation in my whole class discussions. This can be used with 4th grade students and above. Teachers can collect this weekly or bi-weekly to keep students accountable and rewards can be given if students have achieved their goal.
