Perfect, easy & quick form for students to fill out if they have failed a quiz or test and need to do a retake. This helps me keep it organized so on the days we have set aside for retakes, I know exactly who needs to take what so that I am prepared! I print on bright colored paper and have a basket in my room. Students can fill them out throughout the week and don't have to ask to retake, they just fill out the form and turn it in!
