This is a customizable Google Forms quiz designed specifically for middle schoolers in grades 6th through 8th. The quiz includes a detailed list of descriptions of all of the parts of the ukulele that can be used as a separate handout. All questions are multiple-choice (drop-down) questions except for the first two which allow the student to type out the brand of the ukulele and name it whatever they'd like.

This is a quiz on the Elements of Music. I have always included tempo, beat, rhythm, meter, form, harmony, pitch, melody, timbre (tone colour), texture and dynamics when teaching them. You can integrate it into Google Classroom or send your students a link. It can even be done together as a class. Students will watch a short video about the treble staff note names in the form and then take a short "quiz".
