I designed this for myself. I float between three schools as a gifted resource teacher and use this to keep records. I do not have access to my schools electronic gradebook so this serves as how I keep grades and data. This can be modified as needed. I use one each week. There are also spots for notes on each meeting I have with the groups.

We are providing compensatory education in our district due to campuses being closed at the end of the year for COVID-19. This is a simple word document that will make it much easier to document contacts with your students! All the info needed to contact them will be in one place, and you could even copy to front and back! Less paper floating around, less to file in student folders late, and less stress. :)
