I modified our school clinic slip for students in my self-contained special education class. All of my students can independently go to the nurse's clinic. However, they have trouble relaying information to the nurse once they get to the clinic and trouble remembering what to tell me when they come back to class. This form helps to ensure the lines of communication do not get crossed and helps ease frustration of students when they can't fully communicate a problem.
