This is a planning sheet based on 4th/5th Grade common core standards. Students start planning in the first month of school and meet with teacher to set a goal. They should check on their progress and gather evidence for all subjects for a month or longer. A few weeks prior to the conference, students should fill out the middle school ready goals and the subject reflection sheets.
