I usually give this out on the first day of Summer School so that I can get to know the students better. I typically then follow it up with a multiple choice question quiz about myself. I make them guess the answer by looking at me and taking in clues from around the room. I then go over the answers with them and give them some insight as to who I am.

It allows them to reflect on how they felt before coming in, talk about their current feelings, and set goals for the upcoming year.
