I made a checklist to help classroom teachers to address gifted students’ affective needs. I chose questions that I felt could help teachers who may not have experience in dealing with students’ needs. The checklist is to help identify red flags for students who may need some support with their affective needs.

I've used this online grade check form in AVID (as a Google Doc), usually at the end of a grading period, and found it helpful in identifying where students may need support. It's also a good reflection activity, as it asks them to consider their effort and habits when it comes to each class. I make sure to set it up as a support activity so they can feel comfortable in telling me how they're doing.
