This is a planning sheet based on 4th/5th Grade common core standards. Students start planning in the first month of school and meet with teacher to set a goal. They should check on their progress and gather evidence for all subjects for a month or longer. A few weeks prior to the conference, students should fill out the middle school ready goals and the subject reflection sheets. These are in Word, so should be editable based on your specific standards and needs.
