This is something that I experimented with last year in teaching. A lot of times I would send home assessments to keep the parents in the loop of how their child was doing in class and what grades to expect. Well, I was coming to the conclusion that those assessments were not getting home, particularly the ones that were not so hot on the grading scale. This is a form designed to encourage students to mindfully reflect on their test/quiz scores. I give these to my students printed on a colored paper before they recover their test/quiz grade. It allows them to reflect and then I can use it as the cover page for their test/recovery test when scores go home! Great way to encourage parents to discuss with their students about their assessments. EDITABLE so you can change to fit any type of test!
