Coded Weekly Behavior Chart
Use this chart to help set behavioral goals for our students. It can help support daily communication with families, as well as motivate students to achieve their goals. Ex: If a student gets 7/10 positive comments, they earn their chosen reward. After downloading, click on the link to edit the chart in Canva.

Daily Folder Page with Rubric
I made some updates with my previous version of this page. The page stays in the students' take home folder and is a great way to communicate daily student performance to families in a short amount of time. I have several incentives that I connect to this page (adult signatures each night, earning mostly 3s or 4s, homework completion, etc). My favorite part about this page is that students are scored on a rubric that includes attention, behavior, citizenship, and academics.

Individualized Positive Behavior Plan
I used this for two of my students to turn their behavior around! They take ownership for their behavior and it is used as a positive, they want to earn 3 SMILEYS!!! :) There is a page for girls and a page for boys.
At the end of the day, they pick a marker and we talk about if they followed directions, listened, and kept their hands to themselves. They get to color in either the smiley, straight or sad! They love this and it has really worked for these students.
