This is a form I use to monitor student behavior in 15 minute intervals when I go do an observation for a student that has Attention Deficit disorder related behaviors. The form can be modified to check for different behaviors. After I monitor the student I typically pull out the student to my office and discuss what I observed using positive reinforcement and sometimes tangible rewards to encourage correct behaviors.
