This is a great form to record student's testing data, and to provide parents information that let's them know how their student is progressing. I use it for STAR Reading and Math data. After testing, I conference with each student and that child lists a couple of areas that they will work on for the next test. (For ex. This form will help at conferences and end of the year to show student growth. It can be modified to adjust for the assessments and/or times to fit your districts curriculum. My parents really liked having something in front of them showing their child(ren)'s scores and growth. If applicable (such as for MAP scores), I attached a reference sheet explaining where their child(ren) should be at in their learning.
