Are you an upper-grade teacher that hopes to give more accountability to your students during Parent Teacher Conferences? This is a great way to get started. Your students will be very aware of their grades, reading level, struggles, strengths and goals. Students will have this filled out and ready to go prior to sitting down with their parents to give them their own academic details. The bottom of the page gives instructions to meet with the teacher at the end of their student led conference.

Student Observation Log

This is an EXTREMELY simple form I keep in my binders for each class. There is a copy of this form for each student. I keep a clipboard of labels nearby during class, and when I note a behavior that I want to record, or if there is an incident in class, I record it on a label. Later when I have time, I stick the label on the observation form. Now my documentation is a piece of cake. If I need to talk to parents or administrators about a student's behavior in class, I just have to look at my log.
