It provides the following: *Goal being worked on *Date *Activity *Performance (out of 10 for easy record keeping). I use this section for my tally marks *Percentage *Prompting (if any) I usually indicate by writing P for verbal prompting, V for visual prompt, and M for model *Notes section I hope you enjoy!
