Included are 3 different charts to record data on a specific behavior to think of what could be the function of a behavior (not to take place of a Functional Analysis). 2 options are "fill in the blank" type of a chart while the other is a checklist. I included some helpful tips for filling out the different categories, as well as some examples of action steps for each function of behavior. I hope this helps!
