Inclusion Teachers, you will want this document in your toolbox!

This MS Word Form (you will need the desktop application to utilize the Form features) has been a life saver in my co-taught classrooms this year. It lists many of the common classroom/testing accommodations along the top of the form, has check boxes and drop down menu options!!

If it isn't along the top, there is an "other" box you can type in--plus an "other" checkbox up top so you'll be reminded to look below too!
