Looking for a way to learn all about your new students? These All About Me and Interest Inventory forms are the perfect Back to School student forms, as they are an easy, low prep way to discover your students’ self-perception, likes, dislikes, strengths, goals, and more. This is ideal for general back to school, but also for math, reading, and writing - and can be used any time of the year!
This resource is the perfect back-to-school activity to help you learn all about your students and to help them get back into school gear. It is low-prep – simply print and copy OR share the digital version on Google Classroom™ or your LMS. This resource works well as a bell ringer or quiet seatwork on the first few days of school. Hand out to students and then you can do some of the necessary beginning of school tasks like collecting school forms or fees, helping new students, taking attendance or doing lunch count, collecting supplies, etc.
(It is also great to give to new students that come in after the beginning of school.)

Give students the “All About Me” sheets first. This will really allow you to learn about your students’ favorites, learning goals, and about how they perceive themselves.
Give out the Reading, Writing, and Math Interest Inventories during those subjects after you’ve given a brief introduction to the class. These interest inventories will teach you so much about your students and even guide you in how to help them. The inventories push students to be metacognitive and examine their thoughts and behaviors about each subject.
