Use this spreadsheet to track your Tier II behavior intervention program (Check in / Check out, point sheets, etc). This will compile information for each student and includes categories such as attendance, referrals, academic data, office time outs, etc.
The weekly points eared can be entered for each student on their tab and will automatically calculate to identify if they are making positive or negative behavior progress on the overview tab to compare with the other variables and other students in the program.
This is very helpful when trying to diagnose student triggers or functions of specific behaviors. It is also a great tool for parent teacher conferences, as well as gathering information to create a Behavior Intervention Plan and Functional Behavior Assessment.
