Description
This 1-page Behavior Tracking Report is designed for middle school and high school teachers who wish to track a student's behavior in class over a set time. This report is designed to require minimal effort from classroom teachers: all they have to do is fill in a circle (Scantron-style!) to evaluate a student's behavior along a custom gradient. Best of all, the circles from each class can be connected to create a graphical view of a student's progress over an entire day or a week.
Philosophy/How I Use This Product:
Tracking a student's behavior across multiple classes and/or multiple days can be a challenge for several reasons. Firstly, the student must remember to bring the tracking form to each class, as well as submit the form to the supervising teacher at the end of the day or week. Secondly, classroom teachers must remember to fill in the tracking form at the end of a busy class in a detailed and reflective way. This behavior tracking sheet product was designed to make it quick and easy for teachers to complete at the end of their lessons.
For the teacher supervising the behavior tracking, all they have to do at the start is write in basic information about the student (name, day of week, date) as well as describe the specific behavior being tracked (e.g. raising hand to speak, bringing class materials, etc.) so that classroom teachers understand what to observe.
For the classroom teacher, all they have to do to track the behavior is fill in a circle that rates the student's progress towards their behavior goal according to a scale (far below goal, below goal, meeting goal, above goal, far above goal). If desired, the classroom teacher can write additional comments on the right side of the form.
At the end of the tracking period, the tracking teacher examines the completed form and connects the dots to create a clear, graphical representation of the student's progress towards their goal, making it much easier for all parties to visualize, understand, and discuss the student's performance.
Behavior Tracking Report Sections:
Section 1 - Basic Information:
- Student name
- Tracking teacher's name (so student & classroom teachers know who is in charge)
- Day or Week
- Date
Section 2 - Behavior Tracking Chart:
- Chart listing 8 periods vertically (typical of most middle or high school schedules)
- Behavior scale with 5 options (far below, below, meeting, above, far above goal)
- Room for classroom teachers to write additional comments to explain their rating
Section 3 - Final Observations & Communication:
- Space for the teacher responsible for tracking to write summary comments that describes overall patterns in the student's behavior or to record other insights
- Teacher signature (ensures tracking form received from student and evaluated)
- Parent signature (ensures parent-student communication has occurred)
