This resource includes 4 weeks (1 month) worth of data tracking pages to help you get organized and document your math centers. IT CAN BE REUSED as many times as you'd like! It consists of full-color pages as well as a black-and-white printer-friendly version designed to guide you through keeping records and using your data to maintain successful math centers.
The data tracking template includes:
- 2 cute color choices for your cover and weekly dividers
- Student Info page- Use this page to collect assessment data and set goals for each individual student. This gives you an opportunity to make a personalized math profile for each student. Reflect and adjust at least once a month.
- Student Grade Sheet- Use this page to record grades and track assessment data you gather during math centers.
- Math Center Notes- Fill out this page after every small-group instruction session. Reflect on the data you collect and use it to plan future centers.
- Math Center Reflection- Each student will fill out a reflection page after each small-group instruction session. (Editable version included for you to personalize questions.) Use the data you collect to shape and adjust your teacher-led small-group instruction sessions. Make as many copies as needed.
- Digital editable versions of all the above pages on Google Slides.
