Description

Chromebooks and computers are a fantastic addition to the education world, from computerization of tests/quizzes, individualized programs, and educational games that provide instant feedback! However, what are we to do when students misuse the technology? Provide students some control using this restriction conference form, that first allows students to reflect on their actions and consequences. Teachers will then meet with the student to discuss and refine the reflections. This is to help students take accountability for their actions while misusing the technology. On the backside of the sheet is the "teacher's information" form, which allows teachers to quickly check off how the students misused the technology, what number offense it is, and what the conferences are. Space for notes is also provided. Students and teachers will both sign the form so that consequences are understood.

*This is a tried and true form that I use in my classroom of middle school students with a range of emotional and behavioral disorders!
