Use this simple tool to track, record, and identify student needs during conferences. This will allow you the ability to easily locate which students need to be seen, how to personalize your instruction through individual teaching points, and holds both you and the student accountable for what work was done the last time you conferenced. There is a column to identify conference, small group, and even partner conferences as part of your records too. Another way I use this resource is highlighting the names of students I need to see that week as a first priority and identify small groups ahead of time. I use this weekly and keep a collection of these notes for an entire unit of study, based on the month, or even the quarter grading period.
