“By [date], when given manipulatives or visual supports, the student will accurately count from 1–50 in sequential order with 80% accuracy across five opportunities.”

📋 How to Use

  • Record the date and staff name in the first column.
  • Each day includes five opportunities for the student to count from 1–50.
  • Mark a “+” if the student counts from 1–50 without mistakes.
  • Mark a “–” if the student makes an error or skips numbers.
  • At the end of the day, record the total number of correct trials (out of 5) in the “Total” column.
  • Use the Comments box to note attention level, prompts, or supports used.
