Each time my students take a math test I staple their notes and work to the test and this slip and send home for parents to skim, sign, and send back. This way parents are not only aware of the child's grade but also the notes and work the student has done to prepare for the test. Then I keep for my own records in case when parent teacher conferences roll around we need to discuss more about a student. Just add to your drive, change the wording, and print (on bright colored paper to grab attention)! Prints three to a sheet.
