An excellent resource for the beginning days of school (or anytime, really!). This provides a great platform to get students talking about themselves, mathematics, and their mathematical mindsets and histories. I'm always amazed at how much I learn about my students through this exercise.

(I've seen several versions of these floating around the internet. I created my own so I could personalize it a little.)

This was exactly what I was looking for. I was looking for a great get to know you activity to use with my grade 4 students this year that was different than the "math about me" style pages. I wanted something that gave a well rounded snapshot of them as a student even though my focus will just be math and science with them. I printed it out in booklet form and will have them put it in their math interactive notebooks when finished. Even though it is labeled for upper grades, I think grade 4s can easily answer the questions. Can't wait to use it next week.

Sorry!! I didn't see your question until now. Bebas Neue & Barrio. I only use free fonts so you should be able to get them pretty easily!
