Description

These Assessment Reports are something I created to pair with the Eureka Squared Module 4 quizzes and assessments. Each report aligns with the topic quizzes and lists all achievement descriptors for the respective topic. After scoring each quiz, I fill a report out for each student, with areas of need checked. I then attach the report to their quiz as a cover sheet. This helps students and parents understand specific areas of need and aligns that need with specific assessment questions for referencing and future practice.

I save a digital copy of the students' reports and use them as a guide for reteaching, small group instruction, and to inform my RtI work.

I also included an overall assessment report with all standards and achievement descriptors for the whole module. Our district only assesses certain priority standards for each grading period so I included an assessment report with only these priority standards.

The End of Module Rubric was created to report an overall score for the priority standards. I can modify this to fit any district reporting you may need, and am more than willing to tailor it to your specific needs.
