Description

This is and Excel Spread Sheet for Kindergarten through 5th grade that allows teachers, principals, reading specialist or any other school member to track reading data for individual students, whole classes or grades. It has the columns for the following assessments: DIBELS Next, 4 Sight, PSSA, and Phonics Screener. It also has columns for labeling RTII tiers, Subgroups, intervention, and homeroom teacher. There are color coded tabs at the bottom of the spread sheet to navigate through the grade level charts. I have put in 2 example student to model how best use the chart. they can deleted after you download it.

It has automatic color changing rules embedded into the spread sheet. That means when a number/data point is entered the cell changes color to reflect the status of that number. This makes it easy for teacher to know how the student scored and what their next steps for instruction might be. The colors are as follows:

DIBELS Next (FSF, PSF, LNF, NWF, DORF, DORF Accuacy, DAZE)
Red= Intensive, Yellow= Strategic, Green= Core

PSSA and 4 Sight:
Red= Below Basic, Yellow= Basic, Green= Proficient, Blue= Advanced

The Cut scores I created are from the NEW 2012-2013 DIBELS Next Benchmarks and the Members Center for the 4 Sight. Columns can be added if you have more or different assessments you would like to add or change. I used this as my school’s Instructional Coach to track all the students in my school from Kindergarten to 5th grade. I brought it to data meetings and discussed all the students’ data with the teachers and made instructional decisions based on this chart. Teacher loved it and it is already all set up, so all you have to do Is type in your students names and data. It is a great tool for classroom teachers to bring to data meetings. It is a really impressive tool!
