“By [date], when given a visual model or writing prompt, the student will write or copy a complete sentence of 4–5 words using correct spacing, capitalization, and punctuation.”

📋 How to Use
- Record the date and staff name.
- For each weekday (Monday–Friday), provide a visual model or prompt.
- Mark a “+” if the student independently writes or copies a complete, correctly formatted sentence.
- Mark a “–” if the sentence is incomplete, incorrect, or requires more than minimal prompting.
- Use the Comments section to note engagement, prompting levels, or behavior.
- Totals can be calculated out of five daily opportunities for easy progress tracking.
