I created this form to keep track of classes. This way, I can easily reference what classes I've had with which students, what rewards I've used, and what things they have told me. The example student form is not a real student (I looked up a popular name and wrote down hypothetical information). I intend to include the pinyin of names when I can for my actual students because it helps with pronunciation. Calibri font supports this.

Students tell me things they like, dislike, and little details that can mean a lot if they are remembered and mentioned again. I also would like to avoid using the same reward every time.

I included a spot for a photo if you have students that have similar information. I'm not sure that I will use this section but it may be useful so I included the option.

***I recommend printing the first two pages back and front and then printing out more as needed. I have gone up to 99 classes. If you need more, let me know in the comments and I will update the file! ***

**Notes on the files**
This download includes: PNGs of the form, a PDF of the form, and a word doc of the form. Sometimes word can be a little bit glitchy with text boxes and images. If you have trouble, try editing and resaving the example student. This is meant to be a print out but I wanted to provide options.
There is a version with an added date and reward type written in. You can simply write this into the original but I thought I would include both for whatever your preference is.
