I created this form to keep track of classes. This way, I can easily reference what classes I've had with which students, what rewards I've used, and what things they have told me. The example student form is not a real student (I looked up a popular name and wrote down hypothetical information). I intend to include the pinyin of names when I can for my actual students because it helps with pronunciation. Calibri font supports this.
