Description

*IMPORTANT: Risk Assessments should be facilitated by trained school staff such as school counselors and school social workers. This risk assessment is not a stand alone tool and in no way guarantees safety, so always communicate with guardians and mental health professionals when this tool is used. This serves as a collection tool to gather information to support students in need.

The last thing you want to be worried about when you are working with a child at risk is knowing if you are remembering to cover everything during your self-injury or suicide risk assessment. Use this risk assessment form to guide you through conversations with students to collect as much information as you can to move forward to support student safety. This form is a compilation of a variety of forms I've seen over the years, my own ideas and in a way that is organized to flow as a conversation.

Included in this purchase is a 4 page pdf document that includes suicide and self-injury risk assessment questions (including history, triggers, hope, goals, and more), parent contact guidance, and a safety plan template to compete with the student. This form was made to partner with the Columbia Suicide Severity Rating Scale, which is linked within the document in the course of the conversation where it is needed.
