I created one of these a while back, but my sub binder recently went missing in my school and I had to recreate it. I have used this form and subs have filled it out. I also include it as part of my weekly infractions report for my Principal. As a World Language teacher with different classes everyday, I need something that could fit the type of schedule I have and the students I serve.
