In NYS, teachers are expected to keep track of how much instructional time that their students miss. I found this to be extremely difficult so I created a form that is completely editable. This form leaves space for the students name, date, reason missing class, and total minutes lost. I will also be offering this in an excel document so that minutes will be totaled for you.
