I created this unit plan template to use for my grade 5/6 math units this year. It includes space for overall and specific expectations, the learning goal and success criteria, diagnostic and summative tasks, and report card level 3 comments and next steps. The rest of the chart is for planning the lessons, which includes the math problem that will be the lesson focus, assessment planning, and extra practice/homework.
All is laid out in a simple, clear, easy-to-read format that is fully customizable! See the image for an example of the chart when it is completed (pieces from two different units pasted together).
