This is the weekly behavior note that I send home with students. I color in the circle to indicate what color the student ended up on for that day from my Behavior Clip Chart. If a student is green or below, a leave a quick note of the behaviors that the student exhibited to land on that color. If a student gets to orange or red then further actions, notes or call home will occur.

The file did not open very well. The text boxes overlapped the different words which made it difficult to read.
I have converted it to a PDF download, so you should be able to open it correctly now. :)
