I used this kid-friendly progress report to encourage my students to take responsibility for their morning and afternoon behaviors. I designed it to work with a color chart that used clothespins labeled with student names.
This form was hole punched and placed in a folder that stayed in the student's desk all week and was then sent home with current grades to be reviewed and signed by a parent. The folders were collected and checked on Monday. Download the STAR (Students Taking Academic Responsibility) cards that I designed to go on the outside of the folders.
