This is a formative assessment. This prediction piece is used at the beginning of a unit to allow students to see their growth throughout the unit. I ask the students to take the words on the left side of the assessment and put them into a story. The students are then asked to reflect back to this assessment at the end of the unit to measure accuracy. (This most of the time draws a huge laugh from the students.)
This could also be used at the end of the unit to have students reflect over terms learned as a summative assessment. This would be a culminating assessment for the Renaissance unit.
