I send this home in my students' Friday folder every Friday to alert parents of missing assignments. I fill out the line "assignment" with the missing work, and date it only when it has been turned in to alert parents of completion of work. Parents can see if the date is blank that the assignment is still missing. This is a great tool to communicate missing work to parents, and to hold students responsible for returning the assignment.
